Global: Difficult Coversations Across the Continuum

Serious illness conversations should begin upstream as a way to increase patients quality of life and assist with the informed decision making process. This session will be a case based approach to preparing for conversations over the continuum of heart failure care. The focus will be integration of providing basic primary palliative care principles. Additionally, the concept of shared decision making and overview of patient decision aids will be provided.
